Advance Care Planning - It is a way to make sure your key considerations in End of Life Planning are carried out by your selected (1) Medical Power of Attorney (2) Health Care Proxy or (3) A Medical Decision Maker is there to make sure that your last wishes are met. Your information will be stored in a secure digital platform called Thanacare - a copy will be provided. Thanacare is an end of life sponsor of National End-of-Life Doula Alliance (NEDA) and Project S.H.I.F.T, Inc.

Equitable end-of-life is the idea that everyone should have equal access to a full range of options for end-of-life care and planning. Your information will be stored in our secure database  - a copy will be provided. 

Those that are homeless - We discuss options and complete a service request for housing.

Senior isolation - We discuss options in the community to increase socialization, address other concerns you may have and complete a service request.

Education to our low income population and terminally ill. For example many may have repeat visits to the ED for end-of-life care. Need Palliative care support. Together we can work with a Palliative Care team in local hospitals to get the support you need. We discuss completing a service request then discuss options.

Compassionate Communities Model - The Peaceful Presence Project

* Care and housing options for the terminally ill at the end of life - Need our help to navigate the system in knowing resources. I am asking you to help in this endeavour by joining forces and starting a coalition with us in our communities. These individuals are low income or homeless and would have no other home to go to during their last days or months of life. 

For Example: No one should die alone. These are 1homes for the dying in our communities who do not have family members to help them. We have to be their families to help bridge the gap. These are places where they will be staying if they do not have a place of their own. Come one, come all, we together can make a difference in our communities.
